About Heavy Bleeding

Heavy bleeding (menorrhagia) during your menstrual cycle may be your body's way of telling you something is wrong. One of the most frequent complaints women have when coming to the Caring Center for Women is heavy bleeding during their period. These women explain that they have to change their tampon or pad every hour, have to put a towel on the bed when they sleep to not soak through to the mattress, and experience fatigue with cramps, anemia, or pass blood clots when going to the bathroom. For more than 25 years, the doctors at the Caring Center for Women have diagnosed and treated many women with excessive bleeding during their period. They also know that a thorough examination to figure out why it is happening is very necessary. Heavy bleeding can be caused by unbalanced hormones, growths in the uterus, endometriosis, hypothyroidism, pelvic inflammatory disease, kidney or liver dysfunction, and more. Menorrhagia can also be one of the indicators of precancer or cancer of the uterus, cervix, or ovaries. We have several options in treating this condition with medication, intrauterine devices, ablation techniques, or surgery.

Treatment Options

Medication Management

We recommend hormone-releasing medications, such as birth control pills, that deliver progesterone to signal your body to not thicken the lining of the uterus during ovulation. This means that the uterus will have less to shed during the menstrual period. Many patients enjoy the flexibility of being able to stop taking the pill when trying to conceive instead of having a longer-term, hormone-releasing treatment like an intrauterine device. 

Intrauterine Device

A hormone-releasing intrauterine device (IUD) like Mirena can be placed inside of the uterus to help curtail heavy bleeding. While most women associate Mirena with contraception, it is also FDA approved for excessive menstrual bleeding. When the IUD releases levonorgestrel (progestin), the lining of the uterus is thinner. You will have less lining to discharge at the end of your menstrual cycle. 

Ablation Techniques

We offer in-office and outpatient endometrial ablation services with a local anesthetic (cryoablation, NovaSure, and ThermaChoice) to reduce heavy bleeding. Cryoablation is performed in our office using an ultrasound-guided technique to freeze and destroy the lining of the uterus. With radiofrequency energy, NovaSure uses heat to remove the lining of the uterus. ThermaChoice also ablates the lining with heat.

Surgical Intervention

When other options have not been effective, your gynecologist may suggest having a corrective surgery. At the Caring Center for Women, we can treat heavy bleeding with a hysteroscopy, MyoSure, or a minimally invasive robotic hysterectomy. Surgical options are the most aggressive techniques. A hysteroscopy involves the use of a MyoSure hysteroscope inserted into the uterus (through the vagina) so that the surgeon can remove adhesions, fibroids, polyps, and the lining. In this outpatient procedure, the uterus itself is not removed.

We also have da Vinci robot-assisted hysterectomies in a hospital setting under a general anesthetic. This is a laparoscopic surgery that makes its way to remove the entire uterus via several incisions or one incision through the navel.
